Zusammenfassung: | Dissertation, Universitätsmedizin der Universität Greifswald, 2021 Nervus vagus, Postoperative Immunsuppression, Immunsuppression, Postoperativ, Vagotomie, Zellmigration, Surcially induced immune dysfunction Postoperative sepsis with it's possible fatal outcomes was seen after 0,9% of all surgeries between 1997-2006 in the USA. The postoperative immune suppression is one of the main risk factors in this scenario. To examine the postoperative immune suppression our group established the model of Surgically Induced Immune Dysfunction (SID). The Nervus vagus was investigated since the year 2000 due to its ability to modulate cytokyne production during endotoxemia. In this study it is the first time that the influence of the Nervus vagus on the postoperative immune suppression was examined. In detail, the model of SID was combined witha subdiaphragmatic vagotomy (VGX). To examine the cell migration in the postoperative state, leukocyte counts in the intestinal muscularis, the blood and the spleen were measured via flow cytometry. We could show that SID causes a major leukocyte migration, especially lymphocyte migration to the intestines 24h and in case of lymphocytes, also 5d post SID. The VGX reduced this effect almost completely. In the blood lymphocyte levels after SID were significantly reduced compared to controls. The VGX almost counteracts this effect. The spleen as a major reservoir for lymphocytes also showed a strong outflow of lymphocyte populations after SID. Also here, VGX reduced this effect strongly. Cytokine-levels were only elevated for IL6, with no difference between the surgical group 24h and 5d after SID, and TNF-alpha and MCP-1 5d after SID in the non-VGX group ... |
