Background: Strip meniscometry was tested for its suitability for the diagnosis of dry eye disease. It was also examined whether lid-parallel conjunctival folds (LIPCOF) or meibomian gland disease (MGD) have an influence on the measurement result. Material and Methodology: 391 eyes of 201 subjects (85 male, 116 female) aged 18 to 81 years (average age 53.17 ± 16.52 years) were assigned to 3 groups. 54 eyes with diagnosed Keratoconjunctivitis sicca (KCS) and 122 eyes with mild KCS were compared to 225 healthy eyes. Each eye was examined by strip meniscometry, Schirmer’s 1 test, the Jones test, optical coherence tomography (OCT) meniscometry (tear meniscus high - TMH, tear meniscus wide - TMW, tear meniscus area - TMA) and the Ocular Surface Disease Index (OSDI). Each eye was examined additionally for characteristic eyelid changes of MGD. The presence of LIPCOF was detected by OCT. Results: The results of strip meniscometry, Schirmer’s 1 test, the Jones test and OCT meniscometry (TMH, TMW and TMA) were significantly lower (p < .001) in the KCS subjects then in healthy subjects. The OSDI score in KCS subjects was significantly higher (p < .001). The result of strip meniscometry and the degree of KCS correlated with each other (r = -,396, p < .001). The sensitivity of strip meniscometry ranged from 0.79 to 0.89. However, the specificity in the range between 0.42 and 0.5 is unsatisfying.
